Family Medicine Physician – Value-Based Care Model

Practice Overview and Schedule

100% outpatient practice

40-hour work week, including:

36 patient contact hours

4 administrative hours

Clinic hours (subject to change):

Monday, Tuesday and Friday: 8:00 AM - 5:00 PM

Wednesday and Thursday: 8:00 AM - 7:00 PM

Saturday: 8:00 AM - 4:00 PM

One evening shift per week required

Shared call and weekend rotation required (rotated among providers – subject to change)

Patient Care and Clinical Expectations

Practice full-spectrum care for all ages

20- and 40-minute appointments, determined by acuity

Collaborative care model with APP panel sharing (APP supervision required)

Epic EMR

Abridge ambient voice dictation tool

Support and Resources

UpToDate access

Value-based care guides

Medication Therapy Disease Management on-site support

Behavioral health collaboration

24/7 nurse call center

Centralized prescription refill team

Coding education support

Quality data monitoring (Provider Scorecard)

Compensation and Benefits

Compensation – salary ranges from $320k to $375k based on experience

Recruitment incentives up to $250,000, including residency/fellowship stipend (not applicable for internal, lateral transfers)

Continuing medical education – 15 working days and $4,500 CME funds

Paid relocation (not applicable for internal, lateral transfers)
