Nurse (Clinical/ Case Management)

About the Position: This position is located at Army medical Center Madigan, Joint Base Lewis McChord, Washington. A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. This is a Direct Hire Solicitation

Serve as Nurse Case Manager responsible for analyzing problems associated with providing health care to patients and/or beneficiaries for the Military Treatment Facility (MTF).

Coordinate services needed by patients with a variety of complexities and provides intervention when the patient deviates from the anticipated path of progression.

Conduct comprehensive assessments of beneficiary’s health needs to develop a plan of care.

Consult and coordinate with other multi-disciplinary care professionals, agencies, organizations and other ancillary support systems.

Complete follow up assessments in the electronic record, ensuring all updates accurately reflect the patient’s current physical and emotional condition.

Review referrals for accuracy and assists healthcare professionals in referral of patients to appropriate specialty providers.

Educate the patient and members of the healthcare team about case management processes, treatment and healthcare options, community resources, insurance benefits, and relevant psychosocial considerations.
