Physician Assistant - Home Based PC Geriatrics and Extended Care

This position is located within the Veterans Health Administration, Hampton Virginia VAMC, Geriatrics and Extended Care Service, North Battlefield VA Clinic.

Duties include but are not limited to: Utilize clinical assessments to develop plans of care which reflect therapeutic, educational, preventative, and rehabilitative aspects of care. Perform a comprehensive patient assessment including health history, physical examination, evaluations of deviations from the norm and age specific disease screening, immunizations, and health counseling. Formulate differential diagnosis and initiate appropriate intervention based on physical finds. Refer Veterans to specialty clinics when indicated. Advise, teaches, and counsels individuals, families, and groups regarding identified health care needs and provides information for prevention of illness and injury. Provides and documents in the medical record assessments, treatments and patient/family education and information regarding clinical care. Collaborates with other disciplines to ensure continuum of care. Completes a comprehensive and timely discharge summary within the parameters of Medical Center Policy. Discharges patients from care in a timely and effective manner. Places, follows, and acts upon consults and recommendations in a prompt-effective manner.
